Alberta Prenatal Record Form
The Alberta Prenatal Record and Notice of Revisions Alberta Prenatal Record – HS0001-125 (2009/09)
Implementation and availability of the Alberta Prenatal Record:
The revised Alberta Prenatal Record HS0001-125(2009/08) will be available in November/December of 2009. A limited number will be printed to allow for your feedback on any areas of concern that may require adjustments with the next print. The working group is confident that the revisions will meet your approval as a standard for documentation (paper and electronic) of prenatal care in Alberta. Most importantly pregnant women in Alberta attending for prenatal care will have a comprehensive history, risk assessment, investigations and care which will be communicated to other health care professionals involved in their care and that of their newborn throughout pregnancy and birth. Healthy Mother, Healthy Baby Questionnaire Form
The Healthy Mother, Healthy Baby Questionnaire was developed and piloted by the Southern Alberta Child and Health Youth Network. This form is provided as a tool for obtaining information on health determinants that affect pregnancy and baby health.
The Alberta Prenatal Care Providers Guide and Resource for Prenatal Care Providers provides guidelines for documentation on the Alberta Prenatal Record, and the Health Mother, Healthy Baby Questionnaire.

The Notice of Revisions to the Fetal Movement Count Chart (May 2009) and Fetal Movement Count Chart Form HS0001-132 are available. Note that a copy of the Notice of Revision will be provided with each new order of the Fetal Movement Count Chart - Form HS0001-132. The forms will be available from Service Alberta by June 17, 2009 . Ordering information is included on the notice.
